B. SYARAT DAN KETENTUAN PENDAFTARAN

1. PERSYARATAN UMUM

• a. Must be an Indonesian citizen and hold a Pontianak City National Identification Number, verified by a Pontianak City Identity Card.
• b. Must be capable, honest, possess moral integrity, have the competence and a good reputation, and never have been sentenced to imprisonment as per a final court ruling, verified by a valid Police SKCK.
• c. Applicants for the Registration position must be at least 20 years old and not older than 35 years.
• d. Applicants for other positions must be at least 20 years old and not older than 40 years.
• e. Must possess an education qualification that matches the required position, evidenced by original diploma and transcript.
• f. Must have graduated from an institution with accreditation level A/Excellent/B/Good or higher, verified by an accreditation certificate or a screenshot of PDDIKTI/BAN‑PT accreditation of the institution and/or study program at the time of graduation, shown by the graduation date on the diploma.
• g. Must be able to operate a computer and at minimum proficient in Microsoft Office software, demonstrated by a Computer Operation Capability Statement.
• h. Must be physically and mentally fit, verified by a health certificate issued by a hospital or health center.
• i. Must not use narcotics, verified by a Non‑Narcotics Certificate covering six parameters.
• j. Must not be a civil servant and must not have an active employment contract with any other institution.
• k. Must not demand appointment as a civil servant, attested by a 7‑point declaration.
• l. Must never have been dishonorably dismissed from civil servant, contract, or private employment positions, attested by a 7‑point declaration.
• m. Must not be a member or executive of any political party, attested by a 7‑point declaration.
• n. Preference will be given to candidates who have completed linear training related to the applied position.
• o. For health‑care positions, candidates must hold a valid Registration Card (STR) appropriate to the applied position.

3. TAHAPAN PENDAFTARAN

b. Other application methods besides the one in point a are not allowed and will not be processed. Each applicant may only apply for one position according to their educational qualification. All submitted application files become the property of the selection committee and cannot be reclaimed or corrected.

c. The required documents to be uploaded by applicants are color scans of original documents in PDF format and include the following:

1. Cover letter addressed to the Director of UPT RSUD Pontianak Utara, signed and stamped with a 10,000 IDR stamp (attached format).
2. 7‑point declaration letter, signed and stamped with a 10,000 IDR stamp (attached format).
3. Curriculum vitae, signed and stamped with a 10,000 IDR stamp (attached format).
4. Computer operation capability declaration, stating minimum proficiency in Microsoft Office programs, signed and stamped with a 10,000 IDR stamp (attached format).
5. Pontianak City Identity Card.
6. Recent 4×6 cm color passport‑style photo with a red background and wearing a white formal shirt.
7. Diploma corresponding to the applied position.
8. Academic transcript.
9. Certification or screenshot of PDDIKTI/BAN‑PT accreditation of the institution and/or study program at the time of graduation, verified by the graduation date on the diploma.
10. Police Record Certificate (SKCK) issued by the Police with a valid period.
11. Valid Registration Card (STR) for health workers.
12. Valid BTCLS certificate and/or Basic Intensive Care Unit (ICU) certificate, or work certificate from the Hospital Intensive Unit for applicants of the Nurse (Intensive) position.
13. Valid BTCLS certificate and/or Hemodialysis training certificate, or work certificate from the Hospital Hemodialysis Unit for applicants of the Nurse (Hemodialysis) position.
14. Valid BTCLS certificate and work certificate as a nurse in a hospital for applicants of the Nurse (Inpatient) position.
15. Linear‑field training certificate related to the applied position (if any).
16. Work certificate for all positions.
17. Health certificate from a hospital or community health center.
18. Certificate of non‑narcotics use with six parameters.

d. Documents must be uploaded during the registration process according to the specified guidelines. All required documents must be complete; missing files cannot be submitted through any other means. Applicants who upload incomplete or non‑compliant documents will be disqualified.

e. The same stamp may not be used for documents uploaded during the registration process.

4. PELAKSANAAN SELEKSI

a. Selection stages:

1) Administrative Selection – The committee conducts verification and validation of submitted application files. The administrative selection only validates files received within the stipulated period. Incomplete or non‑compliant files are declared not to have passed the administrative selection.

Announcement of successful administrative verification is provided to candidates whose files have been verified. Candidates declared to have passed the administrative selection are those who have met all administrative requirements as stipulated by the Selection Committee and are eligible to proceed to the next selection stage.

Candidates who are declared not to have passed the administrative selection are given the opportunity to appeal, under the condition that they do not correct, modify, re‑upload, or add any documents to those already uploaded.

2) Computer Based Test (CBT) – Candidates who have passed the administrative selection take the Computer Based Test (CBT), consisting of 100 questions with a duration of 60 minutes.

The CBT carries a weight of 80% and comprises:

• a) Basic Competency Test of 50 questions, covering: National Awareness Test (TWK); General Intelligence Test (TIU); and Personal Characteristics Test (TKP).
• b) Field Competency Test of 50 questions, designed according to the technical competence required for the applied position.

The CBT result serves as the basis for determining candidates eligible for the interview stage.

3) Interview Selection – The interview carries a weight of 20% and is attended by candidates who meet the CBT result requirements. Interview participants are determined based on CBT score ranking, with the number of candidates equal to three times the available positions for the applied post. For example, if the available positions are 2, then 6 candidates with the highest CBT scores will be eligible for the interview.

4) Final Selection – The final selection is based on the cumulative score of the CBT (80%) and interview (20%). Candidates declared to have passed the selection are those with the highest cumulative scores, according to the number of positions available for each applied post, without applying a passing grade threshold.

5. PENERIMAAN DAN PENEMPATAN PEGAWAI BLUD

a. Candidates who successfully complete all selection stages are accepted as Employees of UPT BLUD RSUD Pontianak Utara.

b. The announcement of accepted candidates for UPT BLUD RSUD Pontianak Utara will be posted on the official website and social media of UPT RSUD Pontianak Utara.

c. Accepted candidates will undergo the employee acceptance process, consisting of Document Verification and Orientation.

6. KETENTUAN PENUTUP

a. The BLUD employee recruitment selection process does not involve any fees.

b. All applicants are advised not to engage with any offers or communications promising ease of acceptance, question leaks, or access to the UPT RSUD Pontianak Utara or the Selection Committee.

c. UPT RSUD Pontianak Utara is not responsible for any extortion attempts made under its name by unauthorized individuals.

d. Throughout the selection process, applicants must proactively monitor announcements on the official website and social media of UPT RSUD Pontianak Utara. Failure to do so is the applicant’s responsibility.

e. Applicants are not permitted to submit multiple applications.
