PANGANDARAN, Thecuy.com – The HIV/AIDS case in Pangandaran Regency is believed to remain largely undisclosed.

The iceberg phenomenon continues to hinder disease management because some people are thought to be unwilling to get tested until their health starts to deteriorate.

The general practitioner at Padaherang Health Center, dr Hilda Fauzia Akmal, said that the discovery of HIV/AIDS cases so far has shown a pattern similar to the iceberg phenomenon.

The cases detected in health facilities are believed to be only a small fraction of the actual situation on the ground.

According to her, many patients only came for examination when they were already experiencing health complaints.

“Those who come to us first are usually those who already have complaints. Because there are four stages of HIV. Usually at the first stage it hasn’t been felt yet, there are no complaints yet,” she told Radar on Wednesday (7/10/2026).

Hilda said that the average patient who comes to Padaherang Health Center for an HIV test is already in stage 2.

At that stage, symptoms begin to be felt, prompting patients to check their health condition.

After further examination and investigation, she said, several patients were found to have HIV transmission risk factors.

“Those who come to us are probably only a fraction of what actually exists in the field,” she said.

That condition presents its own challenge in efforts to detect cases earlier. Because at the early stage of HIV, the sufferer may not experience any complaints.

Padaherang Health Center itself is a facility that falls under the Care, Support, and Treatment (PDP) category for HIV/AIDS patients.

Patients who have been diagnosed receive treatment as well as accompaniment.

Its management is also continuously evaluated through monitoring and evaluation (monev) from the provincial level.

“Provincial-level monev also often occurs nowadays. So, thank God, it’s increasingly being explored for its treatment,” she said.

For patients whose condition is already severe or has entered stage 4, Padaherang Health Center refers them to RSUD Pandega.

Once the patient’s condition stabilizes and improves, further treatment can be carried out back at the health center.

“So we just refer them to Pandega to stabilize the condition. Then, if the stage has improved and gotten better, they’ll be returned here,” she explained.
